PRINCIPAL DUTIES AND RESPONSIBILITIES:
Provides direct patient care to patients in accordance with physicians’ orders. Identifies factors affecting health care by evaluating the patient’s physical, mental, emotional, environmental and social needs with regard to clinical diagnoses and mental condition.

Candidates MUST have recent home care experience within the last 2 years.
Administers medications and treatments as prescribed by a physician and monitors the patient’s response.
Conducts a comprehensive physical assessment, including the Outcome and Assessment Information Set (OASIS), as required, by obtaining vital signs, monitoring lung sounds, assessing and treating wounds, and reviewing laboratory tests related to problems or illness.
May be required to draw blood and other body fluid specimens for tests and transport to the laboratory; promptly responds to critical values, as necessary.
In conjunction with the attending physician, re-evaluates the patient’s outcomes for care, their continued needs, the effectiveness of services rendered and plans for discharge on a regular basis, altering health care plans accordingly.
Reports and records significant information and changes to the physician in charge, with regard to the patient’s condition, and maintains accurate and timely electronic clinical records to be included in the patient’s medical record.
Responsible for the assignment/supervision of licensed practical nurse and home health aide services provided in the patient’s home
Responsible for increasing the patient’s awareness of medical condition and teaches the prevention of illness and maintenance of general health.
Participates in safety huddles, in-service programs, discharge planning and professional committees.
Provides counseling to patients and their families to meet patient goals and nursing needs.
Adheres to all divisional, corporate, regulatory, and HIPAA requirements related to the privacy and security of confidential patient information.
Follows established priorities to meet deadlines.
Maintains a safe work environment and practices personal safety techniques to prevent injury to self and others.
Complies with corporate and divisional policies and procedures. Assists with orientation and training of new staff members. Performs other related duties and projects as assigned.
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
Requires graduation from an accredited Registered Nurse program. Current licensure by the State of Michigan as a registered nurse required.
A minimum of nine months to one year clinical nursing experience preferred.
The ability to effectively communicate with patients, families and health care personnel is required.
Knowledge of computer use is required to electronically document patient care.
Strong interpersonal and customer service skills required in order to interact effectively with individuals from various cultural and socio-economic backgrounds.
Must possess a current driver's license, a satisfactory driving record and reliable transportation.
Must be able to lift, carry, transport and move clients, if needed.
